Deposit to Book

A 25% non-refundable deposit — calculated on the pre-tax subtotal — is required to secure every appointment. Your card is collected at booking, and your booking is not confirmed until the deposit is received.

Cancellation Policy

Life happens — just tell us early. Cancellations with less than 48 hours' notice forfeit the deposit. Reschedule with more notice and your deposit moves with you.

Appointments beyond 10 miles from Waynesburg, PA carry a $20 travel fee to cover fuel and travel time — plus $20 for each additional 10 miles.

If you or your pet aren't ready at the scheduled time, your appointment may be shortened or cancelled. The deposit is still forfeited and the full service amount may still apply.

The remaining balance is due in full at the time of service. We accept Visa, Mastercard, Discover, American Express, debit cards, Venmo, Apple Pay, PayPal, and tap to pay. Returned or declined payments are subject to a $25 fee.

I confirm my pet is up to date on rabies vaccination as required by Pennsylvania law.
I have disclosed any known behavioral issues, including any history of biting, scratching, or aggression. I understand that Doggy Style Massage LLC reserves the right to stop or refuse service if my pet's behavior poses a safety risk, and I will still be charged per the cancellation policy.
I understand that grooming and massage involve physical handling and the use of sharp instruments. While every precaution is taken, minor nicks, scratches, or stress-related reactions can occur, especially with matted, senior, or anxious pets.
I disclose any known medical conditions, injuries, allergies, or sensitivities my pet has:
I release Doggy Style Massage LLC, its owner, and agents from liability for any injury, illness, or adverse reaction arising from services performed, except in cases of gross negligence.

I understand that severely matted coats may require shave-down, which can reveal pre-existing skin irritation, and I release Doggy Style Massage LLC from liability for conditions uncovered during dematting.

Flea/Tick PolicyInitials required

If fleas or ticks are found, I agree to a flea treatment bath at an additional charge, or Doggy Style Massage LLC may refuse service.

Pet IllnessInitials required

I confirm my pet is not currently showing signs of contagious illness (coughing, vomiting, diarrhea, etc.). I will reschedule if my pet becomes ill before the appointment.

Release of LiabilityInitials required

I release Doggy Style Massage LLC, its owner, and agents from liability for any injury, illness, or adverse reaction arising from services performed, except in cases of gross negligence.

Emergency Vet AuthorizationInitials required

In the event of a medical emergency during service, I authorize Doggy Style Massage LLC to seek emergency veterinary care at my expense.

Photo/Video ReleaseInitials required

3. Service Agreement The booking terms every client agrees to

Booking terms every client agrees to before their appointment. Covers the 25% non-refundable deposit, 48-hour cancellation and no-show charges, the $20 travel fee beyond 10 miles from Waynesburg, and accepted payment methods. Send with the liability waiver at onboarding.
